An area eye surgeon has been indicted by a grand jury on 150 counts of conspiracy, fraudulent schemes, theft and more. Dr. Michael Ham of the Kokopelli Eye Institute was arrested in Prescott Valley Thursday. Ham is accused of fraudulently billing Medicare, the U.S. Department of Veterans Affairs, AHCCSS, as well as several private health insurers for hundreds of thousands of dollars between 2009 and 2018. The Arizona Attorney General’s Office alleges Dr. Ham directed 46 current and former employees to fraudulently complete patient eye exam documents with false information. The information was calibrated in such fashion to satisfy the standards of insurers for cataract or laser surgery coverage. It is possible some patients had surgeries that were not even medically necessary. The institute also has offices in Prescott, Kingman and offices in the Phoenix area. The case is ongoing and remains under investigation.
