The Department of Public Safety has released video of the head-on crash earlier this month on Interstate-40 with a wrong way driver who was impaired. The accident happened on Saturday, November 17th at around 3:30 p.m. Officials say a trooper found the vehicle on I-40 and tried to contact the driver. The vehicle then fled and began driving eastbound in the westbound lanes on I-40 near milepost 195. DPS says a state trooper was in the area and was involved in a head-on crash with the wrong-way driver at milepost 194. The female driver has been identified as 32-year-old Patricia Rose Carvalho and was with her two-year-old daughter at the time. Both were taken to Flagstaff Medical Center for non-life threatening injuries and have been released. The DPS sergeant was also taken to Flagstaff Medical Center for treatment for his non-life threatening injuries and has also been released. Carvalho has been booked on attempted second degree homicide, aggravated assault, endangerment, aggravated DUI and child abuse. The child has been placed in custody of the Arizona Department of Child Services.
